Aged Care Support at Home

Stay independent, safe and active in your own home.

Quinn Health has partnered with Trilogy Care to bring allied health support to older Australians at home. Trilogy Care manages your Support at Home funding, and our clinicians visit you to deliver the care.

What is Support at Home?

Support at Home is the Australian Government's in-home aged care program. It replaced the Home Care Packages Program and Short-Term Restorative Care Programme on 1 November 2025. Clinical care such as nursing and allied health has no participant contribution.

Who is eligible?

You may be eligible for Support at Home if you:

  • are 65 or older, or 50 or older if you are Aboriginal or Torres Strait Islander
  • live at home rather than in residential aged care
  • have been assessed by My Aged Care as needing home care services

Eligibility is decided through a My Aged Care assessment. The assessor looks at things like your daily living capacity, health conditions, mobility and safety, wellbeing, carer supports and personal goals. Not sure if you qualify? You can complete an online eligibility check on the My Aged Care website or call 1800 200 422.

How to get started

  1. Contact My Aged Care. Call 1800 200 422 (Mon–Fri 8am–8pm, Sat 10am–2pm) or apply online at myagedcare.gov.au. A medical professional or hospital can also refer you.
  2. Have your assessment. It takes place in your home, and you can have a support person with you.
  3. Receive your Notice of Decision. This confirms your approved classification and is your formal approval to access funding.
  4. Complete an income and assets assessment. Services Australia carries this out to work out whether you need to contribute to the cost of some services.
  5. Choose Trilogy Care as your provider. Tell them you'd like Quinn Health for your allied health. We'll then contact you to arrange your first visit.

Supporting your aged care assessment

You don't need a letter from your GP or another health professional to apply for an aged care assessment. However, up-to-date health information can help your assessor understand your needs. If you have any of the following, keep them handy for your assessment:

  • A GP letter or health summary covering your diagnoses, medical history and current medications
  • Reports from allied health professionals, such as a physiotherapist, occupational therapist, dietitian, podiatrist or speech pathologist, describing how your health affects your mobility, safety and daily activities
  • Hospital discharge summaries or specialist reports, if you've recently been in hospital or seen a specialist
  • Contact details for your GP and other health professionals, so your assessor can follow up if needed

What helpful information looks like: the most useful letters and reports describe what you can and can't do day to day, such as walking, showering, cooking, managing medications or getting around safely. They also mention any falls, pain, memory changes or safety concerns, and the supports you already have.

Bring to your assessment: your Medicare card and one other form of ID (such as a driver's licence, health care card, passport or DVA card), any referrals, and a support person if you'd like one.

Our team can also provide an allied health assessment or report to help support your application. Contact our office on [phone] or [email] to find out more.
